Start with consequences
List what an action changes and who is affected. Decide which changes can proceed automatically and which need review.
Give the reviewer context
Show the source information, proposed action and reason for escalation. A vague approval button creates work without helping a person decide.
Resolve ambiguity before action
Missing references or conflicting records may need a person to identify the correct context. Make that resolution visible before updating a downstream system.
Keep a useful history
Record the relevant action, review and outcome. Use recurring exceptions to improve the process rather than repeatedly asking people to solve the same issue.
